Once you make an appointment, please print out the first three links under the tab Helpful Forms and fill out as much as you can and bring that along with your insurance card and EAP authorization number, if you have one, to the first session.
When you arrive at the office location, please have a seat in the waiting room just inside the glass entry rather than try to enter my office. You will avoid interrupting the last person's session this way. Thanks!!
